Welcome School of Dentistry residents and fellows! The Office of Student Health Benefits at the University of Minnesota is pleased to administer health benefits and enrollment for School of Dentistry residents and fellows.
Email and U.S. mail correspondence related to your benefits will come from the Office of Student Health Benefits. Please add firstname.lastname@example.org to your email address book to ensure you receive our emails and bookmark this web page.
Open enrollment for 2012 Flexible Spending Accounts for School of Dentistry residents and fellows is November 1 to November 30, 2011. Please see the section titled Flexible Spending Account at the bottom of this web page for more information on FSA enrollment.
Open enrollment for all other School of Dentistry Resident and Fellow Health Benefits has closed. Open enrollment for the 2012-2013 plan year will take place from May 1, 2012 to June 15, 2012.
New residents and fellows must complete the benefit enrollment process for medical and dental benefits within 14 days of their start date.

For those nearing the end of their residency or fellowship—please submit all COBRA election forms at the same time to avoid delays in processing:
Once an applicant’s eligibility is verified by the Office of Student Health Benefits, enrollment will be processed. Plan members will see a charge for the elected plans and long term disability coverage on their student account at the beginning of each term. Member ID cards will be sent to the plan member by U.S. mail approximately three weeks after enrollment. Cards will be mailed to the “current U.S. mail address” on record with the University. To update your address, go to: One Stop > Quick Links > Student Records > Personal Information.
Note: Coverage for residents and fellows continuing residencies or fellowships will remain at the same level of benefits elected for the 2011-2012 plan year unless an enrollment/change form is submitted by June 15, 2011.
Residents and fellows who choose dental coverage through Delta Dental of MN must complete an enrollment form during open enrollment 2011. If you choose not to participate in the dental plan at this time you will not have the opportunity to enroll for coverage until open enrollment in 2013.

Please notify the Office of Student Health Benefits of any change in dependents, or cancellation of coverage by completing and submitting and enrollment/change form or cancellation form. Mailings will be sent to the U.S. Mail address indicated in One Stop. To update your address: go to One Stop > Quick Links > Student Records > Personal Information.
Note: Coverage will be cancelled at the end of the month in which a request is received. In order to cancel Residents and Fellows HealthPartners coverage as a primary member you must also submit a waiver form (PDF).

Guardian Life Insurance Company provides long term disability insurance for School of Dentistry residents and fellows. Enrollment cost for residents and fellows, will be on your student account at the beginning of each term.

To ensure your complete and accurate understanding of several important benefit enhancements, the Office of Student Health Benefits has authorized two Guardian Disability Insurance Specialists to contact University of Minnesota Dental residents and fellows. After Completion—Guaranteed Standard Issue Plan: Optional enrollment
We strongly encourage all School of Dentistry residents and fellows who are completing their residencies and fellowships this spring to contact Dale Forsythe as soon as possible. You have an opportunity to obtain guaranteed disability coverage for life, which is extremely valuable and rare.

You have invested hundreds of thousands of dollars in tuition, fees, and books—and an incredible amount of time—in your education. Through Guardian you have an opportunity to protect that investment should you become disabled. Included in Guardian’s coverage is the opportunity for School of Dentistry residents and fellows to convert to an individual disability policy that you own and take with you—with no medical underwriting—upon completion of your residency or fellowship. This plan offers benefits of up to $3,000 per month should you become disabled. The University is extremely pleased to offer this one-of-a-kind Guaranteed Standard Issue Plan option. University of Minnesota School of Dentistry residents and fellows have the opportunity to enroll in the Guaranteed Standard Issue Plan six months prior to the completion of your residency or fellowship—regardless of any pre-existing medical conditions. 25-30 percent of residents and fellows would not otherwise qualify for this type of coverage due to pre-existing medical conditions. All residents and fellows are required to have health insurance. HealthPartners will provide health plan network and claims administration services for residents and fellows. HealthPartners, in alliance with CIGNA, gives members access to 650,000 healthcare providers and 6,500 hospitals across the United States. You will have a choice of two plans, Basic or Basic Plus. All residents and fellows are required to enroll in one of the two plans for at least single coverage, or provide documentation of other comparable health benefit coverage to the Office of Student Health Benefits. If you have other health insurance coverage, you must complete a waiver form.

School of Dentistry residents and fellows and their dependents who enroll in this HealthPartners plan are automatically enrolled in emergency travel insurance through FrontierMEDEX, a leading provider of international travel assistance services. COBRA Medical: Optional enrollment for School of Dentistry residents and fellows (and dependents) enrolled in HealthPartners
School of Dentistry residents and fellows and their dependents who enroll in this HealthPartners plan are automatically eligible for Continuation of coverage through COBRA at the end of their residency or fellowship. Continuation coverage must be with the same plan option you had as of the date of coverage termination. You do not need to prove that you are insurable to obtain continuation coverage. COBRA coverage is identical to the coverage provided under the plan to similarly situated active residents and fellows, and their eligible dependents. You and your dependent(s) who elect continuation coverage may change coverage options during any open enrollment period that the plan has while you are covered by continuation coverage. COBRA payment and cancellation is administered through Eide Bailly (952) 994-6633.

Delta Dental of MN provides a fully insured dental program for School of Dentistry residents and fellows. Delta Dental members have access to both PPO and Premier providers. The level of benefits are determined by which network is utilized.

School of Dentistry residents and fellows and their dependents who enroll in the Delta Dental plan are automatically eligible for Continuation of care through COBRA at the end of their residency or fellowship. Continuation coverage must be with the same plan option you had as of the date of coverage termination. You do not need to prove that you are insurable to obtain continuation coverage. 2012 Flexible Spending Account Enrollment Form (PDF)—due 11/30/11
Open enrollment for 2012 Flexible Spending Accounts (FSA) runs from November 1 to November 30, 2011. If you would like to take advantage of the FSA program in 2012, you must submit your enrollment form to the Office of Student Health Benefits before November 30, 2011. Failure to enroll during open enrollment will result in the loss of eligibility to enroll in FSA until the next open enrollment period.
New residents and fellows: FSA benefits begin the first of the month following your start date. For example, if your start date is October 3, 2011, your FSA will start November 1, 2011.
Residents and fellows are eligible to participate in two types of Flexible Spending Accounts, the U of M Health Care Reimbursement Account and the Dependent Care Reimbursement Account. Both programs allow you to pay for related expenses using pre-tax dollars.
