Waiving the Plan: 2013-2014 Plan Year Only

You may request to have the University-sponsored Academic Health Center Student Health Benefit Plan (AHC SHBP) waived for two years.

To request a two-year waiver, you must submit the following documentation by fax, mail, e-mail, or in person to the Office of Student Health Benefits:

  1. Waiver Request Form for 2013-2014

  2. Certificate of credible coverage, also called a letter of active coverage, from your insurance company. You must prove you have other coverage by producing a certificate of credible coverage obtained from your managed care organization, insurance company, or a letter to verify your enrollment from your current insurance company.

The deadline to request a waiver from the plan coincides with the University of Minnesota Twin Cities Campus registration deadline each term. The spring 2014 deadline to request a waiver from the plan is February 3, 2014.

The waiver deadline has been extended to February 19, 2014

You are eligible to waive enrollment on this plan if you have health plan coverage for the entirety of each semester.

If you lose coverage during the period for which you have been waived, you must enroll in the AHC SHBP within 31 days of losing coverage by providing proof of involuntary loss of coverage from your insurance company.

You Have Another Health Plan Option

Academic Health Center students may enroll in either the AHC SHBP or the Student Health Benefit Plan. AHC students who elect to enroll in the SHBP for spring semester 2014 must fill out the Waiver Request and Student Health Benefit Plan Enrollment Form and submit it to the Office of Student Health Benefits by February 3, 2014. Note: Academic Health Center students are not able to update health plan information to waive the plan through the University’s web registration system.

The Guardian Long-Term Disability Insurance is provided to all students and may not be waived.

International students in the Academic Health Center are automatically enrolled in the AHC SHBP and are only eligible to waive if enrolled in either the University-sponsored Graduate Assistant Health Plan or a United States-based employer-sponsored group health plan.
