Waiver appeals accepted through Oct. 15

Missed the deadline to waive enrollment on the Student Health Benefit Plan? Appeals are being accepted through October 15, 2014. Please submit a written appeal via letter or email along with a certificate of credible coverage from your insurance company. You may submit your written request and certificate of coverage to our office separately. Please note that an explanation of benefits document, billing statement, benefit summary documents printed from your account on the internet, or photocopy of your insurance card do not count as a Certificate of Coverage. Your coverage will need to be in place for the entirety of the semester to be considered for a waiver.

2014-2015 Student Health Benefit Plan
